Medical Fitness & Responsibility : The parent/guardian confirms that the player is medically fit to participate in football training and
matches. Any existing medical condition, injury, or allergy has been disclosed. The academy is not liable for undisclosed medical issues.
Injury & Risk Acknowledgement : Football is a physical sport and involves the risk of injury. The parent/guardian and player acknowledge
and accept this risk while participating in academy activities.
Emergency Medical Treatment Consent : In case of a medical emergency, the academy is authorised to arrange necessary medical
treatment if the parent/guardian cannot becontacted. All medical expenses shall be borne by the parent/guardian.
Discipline & Code of Conduct: The player agrees to follow the academy’s rules, training discipline, and code of conduct. Repeated
indiscipline or misconduct may result in suspension or termination without fee refund.
Attendance & Commitment: Regular attendance, punctuality, and commitment to training sessions are expected. Prolonged or frequent
absence without prior notice may affect selection, evaluation, or continued participation
Trial & Selection Disclaimer : Admission, batch allocation, and match participation are based on coach evaluation and academy discretion.
Use of Photos & Videos : The academy may use the player’s photographs or videos taken during training, matches, or events for
promotional, marketing, or educational purposes (website, social media, brochures). No monetary compensation will be provided.
Communication : Official communication will be shared via WhatsApp, SMS or notice boards. Parents are responsible for staying informed.
Data Privacy : Player and parent information collected will be used only for academy operations, communication, and development
purposes and will not be shared with third parties without consent.
